Patient Survey 2021-2022
Northlands patients are invited to take part in our Annual Survey which is taking place throughout November and December 2021 and which was compiled in consultation with our Patient Participation Group. Your answers are anonymous and will only be used to improve the Northlands patient experience.

Please do not attend the surgery for your booked appointment if you have a cough or fever. Please ring reception to cancel the appointment or be added to the telephone triage list as appropriate. Please also see NHS 111 website for more information.
Coming to the Surgery
Patients who attend the surgery are requested to wear face masks or face coverings, ie a scarf - the exceptions are children under the age of 11, those with disabilities or those with breathing difficulties. Wherever possible please attend for your appointment on your own.
Owned and run by the NHS, the NHS App is a simple and secure way to access a range of NHS services on your smartphone or tablet. Use the NHS App to:
- get your NHS COVID Pass – view and share your COVID Pass for places in England that have chosen to use this service and travel abroad
- get advice about coronavirus – get information about coronavirus and find out what to do if you think you have it
- order repeat prescriptions - see your available medicines, request a new repeat prescription and choose a pharmacy for your prescriptions to be sent to
- book appointments - search for, book and cancel appointments at your GP surgery, and see details of your upcoming and past appointments
- get health advice - search trusted NHS information and advice on hundreds of conditions and treatments. You can also answer questions to get instant advice or medical help near you
- view your health record - securely access your GP health record, to see information like your allergies and your current and past medicines. If your GP has given you access to your detailed medical record, you can also see information like test results and details of your consultations
- register your organ donation decision - choose to donate some or all of your organs and check your registered decision
- find out how the NHS uses your data - choose if data from your health records is shared for research and planning
Keeping your Data Secure
After you download the app, you will need to set up an NHS login and prove who you are. The app then securely connects to information from your GP surgery. If your device supports fingerprint detection or facial recognition, you can use it to log in to the NHS App each time, instead of using a password and security code. The NHS App is available now on iOS and Android. To use it you must be aged 13 and over and registered with a GP surgery in England.
How to Find your NHS Number
If you do not know your NHS number following this link: https://www.nhs.uk/nhs-services/online-services/find-nhs-number/. You will be asked your name, date of birth and postcode. A text, email or letter will be sent to you with your NHS Number.
Lifting of Covid Restrictions
While many Covid restrictions are due to be removed shortly, NHS England has issued guidance making it clear that surgeries are a high risk setting and should continue with established infection control measures, including face coverings from 19th July 2021.
We are seeing the rates of Covid rise with the more transmissible Delta variant while staff resources are impacted by staff having to isolate.
It is our duty to do all we can to keep our patients and our staff safe whilst we deliver care.
Thank you for your help with this while we work through this new stage of the Covid pandemic.
Booking Your Covid Vaccination
Northlands has broadly completed its vaccination of Cohorts 1-9, ie everybody over 50 and patients aged 16-64 with underlying health conditions. For COVID-19 vaccinations going forwards, please contact the National Booking Service to make an appointment. All local sites are now available through the National Booking Service online or by calling 119 and it's the best way to get an appointment for both 1st and 2nd doses if you don’t have an appointment already. Please don't hesitate, when it's your turn to be vaccinated, please do so as soon as possible. The NHS appreciates that at times the system can be busy and but be assured the NHS will make sure everyone is offered the opportunity to be immunised so please keep trying. New clinics are being set up all the time at various locations including some community pharmacies so your patience is appreciated.
For Cohorts 1-9, Northlands has recorded an uptake of patients receiving two doses of over 90%. This is a fantastic achievement and thank you again to everyone who has helped us, especially our volunteers. We really could not have done it without you.

Of course we’re not just here for when you are unwell. Our team of healthcare professionals and back-up staff offer a number of clinics and services to promote good health and wellbeing whatever your medical condition.
We hope you enjoy having a look around the site and familiarising yourself with some of the online features such as ordering a repeat prescription. The building is suitable for disabled access, including wheelchairs. We have spacious toilet facilities for both sexes off the waiting room and adjacent to the Nurses' treatment rooms.
