Since 1st April 2004 GP Practices do not maintain personal lists for patients/doctors.  All patients are now registered with the practice.  We do however, make a note of the doctor you would prefer to see and endeavour to make appointments for patients accordingly, as this helps in providing continuity of care.  It is better to see the same doctor every time, especially for ongoing problems.  In this way you can get to know your doctor, and your doctor will become familiar with your particular illness and circumstances.  Even an unrelated or minor illness can be important in relation to past illnesses or medication.  However, it may not always be possible to place you with the doctor of your choice as a result of demands on the doctor's time.  On occasion, if a doctor is away, or when we are exceptionally busy, there may be an alternative doctor (known as a locum) at the surgery.  A locum is a fully qualified and experienced GP who is carefully vetted by the practice.

Medical students are occasionally attached to the practice for a few weeks. They are supervised at all times and will not be responsible for treating any patients.

The practice nurses are available by direct access for general nursing services, including advice on minor illness and travel vaccinations, as well as running a well woman service and special clinics for Asthma, COPD, Coronary Heart Disease, Cryotherapy, Diabetes, Hypertension, and Stopping Smoking.  If you would like to see the practice nurse please telephone the appointments number (01249-812141, then press option 1).  If you need to be seen in an emergency you may be seen by one of our nurses first to decide whether to treat you herself or whether you need to see the Duty Doctor.  This is called Nurse Triage.

  Practice Administration

This is carried out by a dedicated team, including secretarial, reception, administration and domestic staff, all of whom have responsibilities for the smooth running of the Practice.

Picture of Receptionists The receptionists are here to help you with enquiries and to make appointments. They are often under great pressure so please try to be patient.
Picture of Secretaries The practice secretaries deal with the correspondence and referrals to hospitals.

The practice also employs four cleaners to keep the surgery and its surroundings clean and tidy and a pleasant place for you to visit.

  Other Health Care Professionals

We also work closely with many other people, including district nurses, midwives, health visitors, physiotherapists, chiropodists, community psychiatric nurses, specialist nurses (e.g. continence, cancer), counsellor, social services and dietician.  There is also a Psychiatric Clinic held at the surgery once a week.

Chiropodists:

A Chiropodist is based at the Health Centre in Broken Cross. You can contact them via 01249-465635. If you need to visit the Chiropodist, ask for a form from the Health Centre, or from Northlands reception. Fill in the form and hand it in. There is no need to see your doctor and your doctor does not have to sign the form unless you are requesting a home visit from the Chiropodist. If you are over sixty, pregnant, diabetic, registered disabled or still at school you will get priority.

Community Psychiatric Nurses:

Community Psychiatric Nurses are based at Rowden Hill House in Chippenham where you can contact them on 01249-456468. Your doctor will put you in touch with them if you need their help.

Counsellors:

A fully trained Counsellor visits Northlands Surgery weekly. Your doctor will refer you to her if she can help you.

Dieticians:

Dieticians are available to Northlands surgery and the Health Centre. Your doctor will arrange an appointment if you need help with a special diet e.g. for diabetes, to lose weight, etc.

District Nurses:

District Nurses are based at the Health Centre in Broken Cross, where you can contact them on 01249-812821. You can also leave messages for them at the surgery, and your doctor will contact them if he or she wants them to visit you.

Family Planning:

An evening Family Planning Clinic is held on the first and third Tuesday of the month at the Health Centre in Broken Cross. You can book an appointment there on 01249-812821.  Family Planning Clinics are also held in nearby towns.

Health Visitors:

Health Visitors are based at the Health Centre in Broken Cross, where you can contact them on 01249-812821. If you have just had a baby the Health Visitor will automatically visit you soon after you come home with your baby.

Midwives:

Midwives are based at the Greenways unit Chippenham Community Hospital where you can contact them on 01249-456434.  When you see your doctor for the first time in a pregnancy you will automatically be put in touch with the midwives.

Physiotherapists:

Physiotherapists are based in Chippenham Community Hospital where you can contact them on 01249-456452.  They regularly visit the Health Centre in Broken Cross.  If you need physiotherapy treatment your doctor will refer you.

Register:

To Register a Birth or a Death, make an appointment on 01249-654361. You will need to take Certificate of Birth or Death with you.

Specialist Nurses:

There are many specialist nurses in this area.  These are fully trained nurses who have extra training to help you with the special nursing problems that can happen with Cancer, Asthma, Eczema, Diabetes, problems with continence, etc.  MacMillan Nurses are specialist nurses who help patients and their families with serious illnesses like cancer.  They are based at Dorothy House in Winsley.  You can contact them on 01225-722988Your GP, Consultant, Practice Nurse or District Nurse will get them to contact you if they can help you.
