a Repeat Prescription |
The form below allows you
to order a repeat prescription to be collected from the surgery or from
your nominated Calne chemist.
Please note that it may
only be used for
repeat items and not
to request items for which you do not have a request slip from your
previous prescription.
You will find it easier if
you have the slip from your last prescription to hand, in order to
correctly describe the items you require. Please
list only the items you require this
time, with strengths
and quantities as they appear on the slip from your last prescription.
Please allow at least two
working days before
collection of your prescription from the surgery or the items from the
chemist.
Please note
that this system uses standard e-mail when transferring your message to
the surgery. The data is not sent through a secure channel and
could, possibly, be viewed by third parties.
